Histiocytosis Occurring in a Patient with B-lineage Acute Lymphoblastic Leukemia

Abstract
Proliferative histiocytosis has rarely been described in association with acute leukemias and other malignancies In most cases, the histiocytosis had progressed rapidly and was fatal to patients. Here the authors report a case with B-lineage
ALL(HLA-DR+, CD10+) and extensive proliferation of histiocytes occurred before cytotoxic chemotherapy. A 47 year old female diagnosed as ALL developed pancytopenia at the fourth postadmission day(#4 day), and bone marrow biopsy(#10 day) showed
that
the
blasts were still present and histiocytes were increased. Then cytotoxic chemotherapy began and general condition of the patient improved. The next bone marrow biopsy(#23 day) showed decreased percentage(1%) of blasts and marked proliferation of
histiocytes which were mostly mature forms Although the nature of histiocytosis could not be determined easily, we considered th the histiocytosis in this patient might be a reactive phenomenon against the sepsis secondary to the pre-existing
leukemia.
The patient died on the 32th admission day in spite of chemotherapy and supportive care.
